Water Baptism Sign-Up

I hereby give New Hope Windward permission to use my likeness and my child's likeness in a photograph, video, or other digital media ("photo") in any and all of its publications, including web-based publications, to be copyrighted and/or used solely for the purposes of New Hope Windward promotional and/or advertising material, website, and publications, whether print and/or electronic, which includes provision to include statements, such as testimonials, quotes, without payment or other consideration. I understand and agree that all photos will become the property of New Hope Windward and will not be returned. I hereby irrevocably authorize New Hope Windward to edit, copy, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo/video. I hereby hold harmless, release, and forever discharge New Hope Windward from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other person acting on my behalf or on the behalf of my estate have or may have by reason of this authorization. This release shall remain in effect for each child until that child reaches legal age.

By attending this event you recognize you may come into contact with someone at this event who had COVID-19 and you may contract the virus yourself. You also agree that you hold New Hope Windward, its staff, volunteers HARMLESS, if you contract COVID-19 or any other disease while attending the event.

By electronically signing and submitting to this agreement, I agree that my electronic signature is the legal binding equivalent of my handwritten signature on paper. I will not at anytime, claim that my electronic signature is not legally binding or enforceable. I acknowledge that I have read and fully understand the terms of this agreement. I understand that if I wish to sign a hardcopy agreement, I must contact New Hope Windward that requires my signature on this agreement directly.

A parent or legal guardian must sign this agreement on or behalf of any minor participant under the age of 18. By electronically signing this agreement, I represent that I have the authority to sign and enter into this agreement on behalf of the minor(s) listed above.
